Informed Consent Form for Teleconsultation – Bioplus – Saúde e Educação Lda
The World Health Organisation (WHO) defines telemedicine as the remote provision of healthcare by professionals using information and communication technologies (ICT) to enable prevention, assessment, diagnosis, treatment and rehabilitation, as well as the continuous training of healthcare providers, in order to promote the health of citizens and society. This is done via smartphone, tablet or computer with an internet connection.
Bioplus – Saúde e Educação, Lda is a recognised entity for conducting these consultations (Rua César de Oliveira 14A, 1600-427 Lisbon) – (ERS No.: E172857)
I declare that I am a customer of Bioplus, which invited me electronically to access a medical consultation (teleconsultation) via telemedicine.
It is my free choice to use telemedicine via videoconference.
I understand that the teleconsultation is individual and non-transferable, and will allow me to present to my healthcare professional: clinical information, signs and symptoms, results of complementary tests, medications I am taking, so that they can perform the procedures necessary for the purpose of my consultation.
I understand that my physical presence at the clinic may be necessary.
I understand that: (i) the video images and audio from the teleconsultation are viewed by the healthcare professional; (ii) the chat text will not be recorded or stored; (iii) the healthcare professional is responsible for ending the teleconsultation; (iv) my personal information will be stored in a secure environment, maintaining the confidentiality of my personal data, to which only necessary and duly authorised persons will have access; (v) my data will not be shared with unauthorised third parties under any circumstances without my prior express and unequivocal authorisation; (vi) I must be in a private location with good lighting/acoustics and without interference.
I am solely responsible for the technical conditions of access for the teleconsultation by videoconference with regard to: equipment, internet availability through my server, and video and audio functioning, and I undertake to test all these aspects before the start of the consultation and as soon as I receive the link, so that I have sufficient time to make any necessary repairs or adjustments.
I understand that teleconsultation via videoconferencing has both risks and benefits. The benefits are related to not having to travel, the speed of the consultation, and the possibility of clarifying questions about my health. The risks are related to the absence of a clinical (physical) examination, which may require the consultation to be converted to an in-person consultation, at the discretion of the healthcare professional, or that I have to undergo tests to supplement the information I have provided.
Finally, I agree and authorise the practice of telemedicine to be carried out by the healthcare professional. I authorise access to my personal data by Bioplus (e.g. National Health Service number, identification document, driving licence, among others necessary for the purposes of the service I have purchased) and health data available to the doctor who will carry out the consultation, for the sole purpose of providing appropriate care.
I confirm that I have read, understood, and agree to these Terms and Conditions for teleconsultation services provided by Bioplus – Saúde e Educação, Lda.
I am aware that any questions I may have can be answered by Bioplus customer service.

Finally, I am aware that I may not grant this consent or grant and revoke it (uncheck the checkbox) at any time before entering the teleconsultation, and that if it is not granted or is revoked, I will not have access to the video consultation I had scheduled.
